RMS Registration Forms and Documents

Once you have downloaded and filled in the forms and documents to the right of the page (RMS FORMS & DOCUMENTS), you must submit them during the online registration process in order for your application to be accepted online. You may also contact our office by emailing amarple@riverviewmemorialschool.org for more information about the documents you'll need to process your application.

There are three major processes needed to apply and register for Riverview:

MEDICAL CONSENT—MEDICAL: I do hereby consent to any x-ray examination, anesthetic, medical or surgical diagnosis or treatment and hospital service that may be rendered to said minor under the general or special instruction of the above-named physician or any physician the school may call.  FIRST AID: I do hereby consent to reasonable and prudent first aid to be administered by school personnel to the said minor as circumstances merit.

STUDENT—I am aware of the student behavior expectations as presented in the Riverview Memorial School (RMS) handbook. I pledge to obey all printed and announced regulations of RMS. I realize that I am subject to disciplinary action if I violate this pledge. Furthermore, I solemnly pledge to uphold the Christian principles upon which RMS is founded and to perform to the best of my ability all curricular and extracurricular activities assigned to me.

PARENT/GUARDIAN—If my child(ren) is accepted for enrollment at Riverview Memorial School (RMS), I understand that continuing enrollment is contingent upon complying with the policies and procedures stated in the RMS handbook and that my child is subject to disciplinary action if he/she fails to fulfill this agreement. By selecting ACCEPT, I agree to abide by the policies and procedures stated in the RMS handbook. (An electronic copy of the handbook is available upon request and on our website).

RMS ONLINE REGISTRATION

ONLINE registration is now open! During the ONLINE registration process, you will need to download the PDF documents and forms below, fill them out, and return them to our offices by uploading the completed documents & forms at the bottom of this page under the heading "UPLOAD SIGNED DOCUMENTS IN THIS LOCATION" on or before Sept. 4.
